Microsoft Excel: zipcode merge problem

When I do mailmerge for labels with Word, a few of my zipcodes are there
(randomly) but most are not. What am I doing wrong? Also, sometimes I get
to choose F1, F2, etc. for the fields, and sometimes I get First name, Last
name, etc. How do I get that to be consistent each time I merge? This is
giving me fits.
